Purpose:
Members will be aware that Welsh Government developed a one year ‘Intermediate Care Fund’ (ICF) to encourage integrated working between local authorities, (including Housing and Social Care), Health and other partners.
This report provides Cabinet with an overview of the projects delivered by the Intermediate Care Fund and the opportunity to scrutinise the outcomes that have been delivered.
